Customize toolbar -bluescreen of death

When I try to customize the toolbar I get a blue screen.
I was getting a blue screen before using SlimJet with a NetGear A6210 adapter.
Switched to a TPlink and did not see a blue screen until this happened.
Both of these wifi adapters use USB 3.0.
Tried multiple times and always get the blue screen.
Searched the net and can't find anything on this.
Really enjoying using slimjet and hopefully can find out what causes the blue screen.
I was going to add a custom button to allow view source of web pages.
Windows 7 Ultimate 32-bit
32 gigs of memory
